Description: Obesity treatment requires collaboration among multiple different teams. The key for successful treatment and outcomes is to connect patient to primary care providers and specialists in a comprehensive, inter-disciplinary, evidence-based approach to address obesity as a chronic disease and change the way we deliver health care. Come to this session to learn how we can all reshape the future of health care, empowering patients and providers. You will learn not only from the experts but also from the patient and community perspective. Topics will include how to educate primary care providers and patients, the creation of a primary care delivery model, and how to reengineer health care delivery to address chronic diseases.
